- Annual gross income of both parents must not exceed ₱300,000

- Must demonstrate leadership potential
- Must not be receiving any other scholarship grant

- Birth Certificate issued by PSA or NSO
- Senior High School Report Card (Grade 12 – 1st Semester grades)

- Parents’ latest BIR Income Tax Return (ITR) or Tax Exemption Certificate
- Barangay Clearance
- One (1) recent 2×2 colored picture
